Vitamin B12 Deficiency Anaemia

Macrocytic (megaloblastic) anemia from inadequate B12 (cobalamin) for DNA synthesis in erythropoiesis, also critically required for neurological myelin maintenance : neurological complications can occur independent of and even precede hematological changes.

Causes: pernicious anemia (autoimmune, anti-intrinsic factor/anti-parietal cell antibodies, impaired absorption; most common cause in developed countries), dietary insufficiency (strict vegans without supplementation), malabsorption (terminal ileal disease ; Crohn's, resection; celiac disease; chronic pancreatitis), post-bariatric surgery/gastrectomy, prolonged metformin use, PPI/H2RA long-term use (reduced acid impairs release from food), Helicobacter pylori infection, nitrous oxide exposure/abuse (inactivates B12).

Iron Deficiency Anaemia

Most common cause of anemia worldwide, resulting from inadequate iron for hemoglobin synthesis due to insufficient intake, malabsorption, increased demand, or most importantly in adults; chronic blood loss.

Microcytic, hypochromic anemia. In adult males and postmenopausal women, iron deficiency is presumed to be from GI blood loss until proven otherwise, given malignancy risk.

Etiologies:

  • Blood loss: GI (peptic ulcer, malignancy, angiodysplasia, IBD, hookworm/parasitic infection, hemorrhoids, esophageal varices), menstrual (menorrhagia), other (urinary, recurrent epistaxis, blood donation)

  • Increased demand: pregnancy, infancy/rapid growth, lactation

  • Decreased intake/absorption: dietary insufficiency (vegetarian/vegan without supplementation, poverty/malnutrition), malabsorption (celiac disease, atrophic gastritis, post-gastrectomy/bariatric surgery, H. pylori infection reducing absorption)
