qSOFA

qSOFA does not exclude sepsis. If infection is suspected, continue to assess clinically and reassess — a score below 2 should never be used to rule out a deteriorating patient.

qSOFA is a prognostic prompt, not a diagnostic test for sepsis and not a screening tool on its own. It was derived to identify patients with suspected infection at higher risk of poor outcome, and it is deliberately insensitive early — many patients who go on to deteriorate score 0 or 1 at first contact. Treat clinical concern as the trigger to act, whatever the score.
