Chronic Cholecystitis

Chronic cholecystitis is chronic inflammatory and fibrotic remodelling of the gallbladder, usually caused by repeated episodes of gallstone related cystic duct obstruction.

The gallbladder becomes thickened, fibrotic and poorly contractile. Advanced disease may produce a small contracted gallbladder densely adherent to surrounding structures.

Chronic cholecystitis is usually the pathological substrate behind recurrent symptomatic gallstone disease rather than a distinct acute inflammatory syndrome.

The major surgical importance is that longstanding fibrosis can make identification of biliary anatomy difficult and increase bile duct injury risk.

Mirizzi Syndrome

Also known as: Mirizzi's syndrome

Mirizzi syndrome is obstruction of the common hepatic duct or common bile duct caused by an impacted gallstone in the gallbladder neck, Hartmann pouch or cystic duct.

Chronic inflammation can erode into the bile duct and create a cholecystobiliary fistula.

The major operative danger is severe distortion of Calot triangle with a markedly increased risk of common bile duct injury.

Clinically useful progression is:

  • External bile duct compression without fistula
  • Partial cholecystobiliary fistula
  • Increasing destruction of the bile duct wall
  • Complete or near complete bile duct wall loss
