Oesophageal Cancer

Also known as: Esophageal cancer

Malignancy of the esophagus, divided into two histological types with distinct risk factors and geography.

  • Squamous cell carcinoma arises in the upper and mid esophagus, is strongly linked to smoking and alcohol use, and remains the dominant subtype in high incidence regions of Asia and East Africa.

  • Adenocarcinoma arises in the lower esophagus and gastroesophageal junction from Barrett's metaplasia secondary to chronic GERD, and is now the dominant subtype in Western countries, closely associated with obesity and reflux disease.

Enteroatmospheric Fistula

An enteroatmospheric fistula is an abnormal opening of exposed bowel directly into an open abdominal wound.

Unlike a conventional enterocutaneous fistula, there is no mature soft tissue tract between bowel and skin. The exposed bowel communicates directly with the atmosphere.

This distinction is critical because spontaneous closure is exceptionally uncommon. The surrounding open abdomen is often inflamed, densely adherent and unsuitable for early definitive surgery.

Common settings include prolonged open abdomen management, abdominal sepsis, repeated laparotomy, bowel injury and direct trauma to exposed bowel.
