Umbilical Hernia

An adult umbilical hernia protrudes through or immediately adjacent to the umbilical ring.

Adult disease is usually acquired and is associated with obesity, pregnancy, increased intra abdominal pressure, ascites and progressive weakening of the linea alba.

The sac commonly contains preperitoneal fat or omentum and may contain small bowel.

Small defects may have a relatively narrow neck compared with the volume of herniated contents and can therefore incarcerate.

Incarcerated Hernia

An incarcerated hernia is a hernia whose contents cannot be reduced back into the abdominal cavity.

Incarceration does not automatically mean the bowel is ischaemic.

The distinction from strangulation is critical:

Incarceration means irreducibility.

Strangulation means compromised blood supply.

An incarcerated hernia can cause mechanical bowel obstruction and may subsequently progress to strangulation.
